Medical
Four medical insurance plans are offered as of the 1st of the month following
60 days of employment:
Dental
Direct Dental Reimbursement- Reimburses you and your covered dependents
at 50% until a $1500 yearly benefit is paid.
Vision
Anyone covered under one of the three NBA medical plans is automatically enrolled
in EyeMed. This voluntary vision program can be used at locations nationwide
for discounts on eye exams, frames, lenses and contact lenses. Vision coverage
is 100% company paid.
Life Insurance
Zions provides basic coverage of one times an employee's annual salary, rounded
to the nearest $1,000 to a maximum of $250,000. Employees may purchase additional
coverage for themselves up to three times their annual salary to a maximum
of $300,000 with no health questionnaire. They may purchase up to $1,000,000
in coverage, not to exceed ten times an employee's annual salary with a health
questionnaire required. Benefits are paid to the beneficiary (ies) designated
by the employee.
Dependent Life Insurance
Employees may purchase supplemental life insurance for their spouse up to
$1,000,000 in coverage, not to exceed ten times the employee's annual salary.
Supplemental life insurance can be purchased for an employee's dependent children
in an amount of either $5,000 or $10,000.
Accidental Death and Dismemberment Insurance (AD&D)
Employees can elect AD&D insurance for themselves and family in increments
of $25,000 up to $750,000 maximum. Any amount over $250,000 cannot exceed
ten times the employee's previous year earnings. The employee is covered at
100%, spouse at 60% and each covered child at 25% of the elected coverage.
Long Term Disability Insurance (LTD)
Employees must be full-time, 25 years of age, working 40 hours per week, and
have two full calendar months of employment to be eligible for this plan.
LTD insurance is provided at no charge to employees and is equal to 60 % of
the employee's regular earnings to a maximum of $8,000 per month, and payable
after a 90-day elimination period.
Long Term Care Insurance
Unum Provident offers Long Term Care insurance
for employees, and employees are responsible for 100% of the cost for this
coverage.
Health Savings Account
(Only available to participants in a HDHP)
Flexible Spending Accounts (FSA)
Two pre-tax salary reduction savings plans are available to help pay for certain
health care and dependent care expenses:
· Health Care FSA: Allows a maximum of $5,000 per year to cover certain
medical, dental, vision, hearing and other eligible health care expenses that
are not covered by another health care plan. Eligible expenses include co-payments,
deductibles and amounts you pay over usual, reasonable and customary limits.
· Dependent Care FSA: Allows a maximum of $5,000 per year to pay for
dependent care for children up to age 13, and for qualifying older dependents.
Vacation
Two to four weeks of vacation are available to employees who are regularly
scheduled to work 20 or more hours per week. Vacation is accrued based on
years of service, salary grade, and hours worked.
Holidays
The bank observes paid holidays as published and observed by the Federal Reserve
Bank (normally 10 per year.) The following are standard bank holidays:

Sick Leave
Sick days are accrued at the rate of one day per month, to a maximum of twelve
(12) days per year. There is no lifetime maximum for sick leave accrual. A floating
holiday will be granted in the year following any full year during which NO sick
days were used. Family leave of absence is available for bona fide medical reasons,
including pregnancy and pregnancy-related conditions; birth or adoption of a
child; or the need to care for you, a child, spouse or parent with a serious
health condition.
401(k)/ESOP
The Zions Bancorporation 401(k) Plan is available to employees who are 21
years or older, immediately upon hire. Employees who participated in
a 401(k) plan at their previous employer may roll over their 401(k) balance
to Zions Bancorporation's 401(k) plan immediately upon hire. Employees
can contribute up to 15% of earnings; Zions will match 100% of the first 3%
the employee contributes, and 50% of the next 2%, for a total match of 4%
to your 5% contribution.
The company may make a discretionary Profit Sharing contribution to the
plan each year, automatically invested in Zions Bancorporation common stock. To
be eligible to receive the Profit Sharing contribution, employees must be
age 21 or order, earned 1,000 hours of service during the prior calendar
year, and be actively employed on the last day of the year.
You're The Owner
You're The Owner is Zions' broad-based, non-qualified stock option plan.
It offers eligible employees the opportunity to share in the company's
success through stock options without having to invest any of their own
money. Employees are granted stock options based on salary grade and are
eligible if they have been employed by Zions (or a Zions subsidiary) for
at least one year and are regularly scheduled to work more than 20 hours
a week at the time of the option grant.
Employee Assistance Plan
Confidential, professional short-term intervention counseling services are
available to employees and their immediate families at no charge.
Educational Development
NBA provides education assistance to qualified, full-time employees to develop
a highly skilled and well-educated workforce. The Education Assistance Programs
can help in achieving professional development goals. Eligible education
programs include workshops, seminars, single classes, or degree programs
that are not offered in-house. No repayment of tuition reimbursement is required
if the employee remains with the bank for four years after payment of the
reimbursement.
Employee Banking Benefits
Employees of the bank are eligible for a number of banking benefits. In addition
to one personal checking account, employees can open unlimited personal savings
accounts free of charge. Wallet-style and duplicate checks are free-of-charge
for the free checking account, as are standard traveler's checks, money orders,
online banking, and out-going wire transfers. All employee consumer loans
are discounted by 1%, regardless of the loan amount and the number of loans.
